Hello,

My name is Beth, the Insurance Coordinator at Clearwater Dental Associates, and here are some important notes to consider.

When calling the office to schedule a first time appointment, or in simple need of updating your insurance benefits, please have the following information:

  • Insurance company name
  • Employer name (if applicable)
  • Benefit or provider telephone number
  • Member Identification number
  • Group number

If you are unable to provide this information please contact your Humana Resources Department or Insurance Broker. I am unable to verify insurance without that material.

We welcome all patients to our practice. I like to inform future and current patients that the staff will work with you regarding insurance. Below is a comprehensive list of insurance companies that each Doctor is in network with.

A doctor that is not in network with your insurance company does not mean that you cannot be seen at our practice or by the doctor you wish to see. As long as your plan offers out of network benefits the doctors can still meet your dental needs.

*Unfortunately, none of our doctors are participating with any DMO, HMO or Medicaid dental plans. If you have any of those plans, please contact your insurance company to find out who will be in your dental network.

ARE YOU LOOKING TO PURCHASE A PRIVATE DENTAL POLICY?

If you are a patient that is interested in seeking a self-funded policy, we have some instructions for you.

First I ask you to weigh the cost of your monthly premium versus your average yearly out of pocket expense. Sometimes paying the monthly premium can be more costly than paying for you normal hygiene visit and yearly exams. If a self-funded policy is still the direction you are leaning towards, I urge you to ask the Insurance Broker the following questions:

  • Is there a waiting period for basic or major services? Some plans offer benefits but make you pay the monthly premium anywhere from six months to a year before they will remit payment for any of those services.
  • Is there a missing tooth clause? Most patient are seeking help with major coverage when purchasing self-funded policies. If part of your plan is to have teeth replaced with this new policy, please ask this question. If a missing tooth clause is in place the policy will not pay to replace any teeth that were missing prior to the effective date of the policy. So that Partial you were looking to get to replace those molars that have been missing for a few years do not expect the policy with a missing tooth clause to pay for it.
  • What is the annual maximum? If the policy only offers a $1000 calendar year maximum and your premium is $800 for the total year is this cost effective?
