The fastest and easiest way to schedule your appointment with one of our doctors is to contact our Clearwater, Florida office at (727) 797-8800. You can also email us at cda@knology.net .
One of our staff members will promptly schedule you with a convenient time to see the doctor. For your convenience we also have an office map and directions available for you to print out below.
We value your time. You will be pleased to find that we will generally see you right at the appointed time. We will make every effort to complete your treatment is as few a number of visits as possible.
In order for us to be efficient with the time that we have scheduled for you, we would ask that you also be prompt and always give 24 hours notice if you are unable to keep an appointment. We greatly appreciate that courtesy as other patients on our waiting list may be served.

Insurance Plans
Dental insurance PPO plans are designed to offset the cost of your dental treatment, they do not cover 100% off all dental treatment. As a courtesy, we will be happy to file your insurance claim for you. Whenever possible we file dental claims electronically to expedite the payment of the claim.
Dr. Burton is a contracted provider with Delta Dental Premiere and Asssurant. Dr. Hernandez is a contracted provider with Assurant, Cigna PPO, Delta Dental Premiere, DeCare, Metlife, Principal, Reliance Standard and United Concordia.
Unless otherwise arranged, you will be responsible for payment in full of the estimated out of pocket portion for your dental treatment at the time of your visit. We accept checks, cash, Visa, Master Card, American Express and Discover for payment.
If you need additional financing, we also offer extended financing options through Care Credit or Florida Central Credit Union. Both companies offer several different financing options enabling you to arrange a financing schedule that best suits your needs, while completing your necessary dental treatment.
Payment Options
Ability to pay is of deep concern to many dental patients. We share your concern and are prepared to offer you a number of payment options. We will discuss the payment plan best suited for you in privacy and assist you with all the arrangements.
If you have dental insurance, we file claims as a courtesy for our patients. Payment is typically due at each appointment, as well as any fees not covered by insurance.
We accept cash, checks and most major credit cards. Flexible financing plans are available. We will be glad to discuss and financial concerns you have, and try our best to work out a financial plan to meet your needs
